The medical record, the official document of admission, contains information on the anamnesis, clinical objectivity, diagnostic procedures and therapy performed during admission.

The Head of the medical records are the persons in charge of the medical records and, as far as they are concerned, the doctors of the department.

A copy of the medical record can be requested by completing the form at the Acceptance - Secretariat, or, pursuant to the law on self-certification, by sending a request by post with motivated reasons.

The copy of the medical record can be withdrawn by the person concerned or by another person who presents a written proxy or is a person entitled (person exercising parental authority, guardian, curator, attending physician).

The entire documentation is available to the patient within 45 days from the presentation of the request; the same procedure is provided for reports of diagnostic examinations and any copies requested at later times.
